David Streeter has passed away. He never recovered from a fall that happened in August (iirc). He passed on December 26.

When trains meet.

Two trains collide in Indonesia's West Java province, killing at least four people.

Two trains have collided on Indonesia's main island of Java, causing carriages to buckle and overturn killing at least four people.

The accident happened about 500 metres from Cicalengka train station in West Java province, said Ayep Hanapi, a spokesperson for PT Kereta Api Indonesia, the national railways.

He said the Turangga express train carrying 287 passengers was travelling from Surabaya, the capital of East Java province, when it ploughed into a Bandung Raya commuter train with 191 passengers heading to Padalarang from Cicalengka station at about 6.30am (local time) on Friday.

"All passengers of the two crashed trains have been evacuated safely," Mr Hanapi said, adding that 28 people were injured.

He said 22 of them were being treated at a hospital, mostly with head injuries.

West Java Police spokesperson Ibrahim Tompo said at least four train crew members were killed — the driver and his assistant on the commuter train and a steward and a security guard on the express train...

I still have the 2 books that came with 3.11 in my library, but I gave away the 2 complete box sets that I had (along with complete box sets of '95 and '98SE on floppies) to an avid young fellow down the road who also takes all the old hardware that I can't be bothered with anymore.

Court finds that derailment was completely avoidable if communications hadn't broken down.

Australian Rail Track Corporation and NSW Trains plead guilty to duty breach after fatal Wallan derailment.

Two government rail organisations have pleaded guilty to criminal charges after a deadly high-speed train derailment in Victoria.

The XPT train service between Sydney and Melbourne derailed on February 20, 2020, near the regional town of Wallan with 155 passengers on board.

The train driver, John Kennedy, 54, and his co-pilot, Sam Meintanis, 49, were killed in the crash, while 61 passengers and five staff were injured.

The Australian Rail Track Corporation (ARTC) and NSW Trains each appeared in the Melbourne Magistrates’ Court on Friday, pleading guilty to a breach of duty under the Rail Safety National Law.

The court was told the train had been diverted along a secondary track, entering at more than 100km per hour when the speed limit was just 15km per hour.

This caused the lead power car to roll and all five passenger cars derailed in a “concertina effect”.

Lawyers acting for each government-owned organisation acknowledged they had breached the law and spoke of the “deep regret” felt......

,,,,,,Under the system, a qualified worker would board the train between Kilmore East and Donnybrook, and advise the driver of issues along the line.

However in the days before the crash, the ARTC made a number of changes to the safety system and procedures.

One of these changes, implemented on February 19 would divert the XPT train service along a secondary 1.5km line from 2.30pm the following day, however the information was not passed along to Mr Kennedy or NSW Trains.

The court was told Mr Kennedy had driven the route eight times between February 6 and 20, with this being the first time he was diverted. ....

Not a new idea, the crossing at Sumburgh Airport has been in place since before 1975, which is when I first used it.

And then there's Buochs Airport in Switzerland where Pilatus have their factory, although there have traffic lights for the aircraft & the road traffic.
