1) Ah, bought from the pirate store. Er, since it is no longer available (at least with a USA IP address) it does not indicate who the seller is/was.

On the USA Amazon site, I bought some medical supplies that arrived marked for sale in the UK only but they are fulfilled from an Amazon warehouse a few miles from my abode. I knew at least the item is identical as the USA, just the patient information sheet is different, about five pages less legal crap.

2) Er, sometimes they can't rent a world wide copyright release, because different people own the rights in different countries.
