If she only meant to user her taser then that sounds like very poor training and positioning of the gun and taser on the kit. The two weapons should be positioned to keep them well physically separated, and training used to make sure that they are no such mistakes. Plus of course the feel of the two weapons should be very different, again different feel. A whole lot of errors in that even before we get to the shooting itself.

Other things I noted in news reports;

The pistols have a trigger safety and the tazers do not.

Brooklyn Center Police Department protocol dictates that officers wear their guns on their dominant side and Tasers on the opposite side of their bodies, to reduce the risk that they will confuse the two weapons. But this doesn't stop them having the tazers in a cross draw position so they can be used with the dominant hand.

“all Taser devices shall be clearly and distinctly marked to differentiate them from the duty weapon and any other device.”

The device should not be used against people “whose position or activity may result in collateral injury” — including people who are “operating vehicles.” Mr. Wright was sitting in the driver’s seat and his car traveled several blocks after he was shot.

IMO it all points to a lack of training.

depends on which firearm is used/issued by this police department. I know many departments like to issue Glock model pistols, and while these technically have a safety, it's a safety that is built into the trigger assembly and the act of pulling the trigger with your finger releases the safety. it basically only prevents unintended trigger activation from something like a drop or other outside forces other than a finger pulling back on the trigger. If you put your finger on the trigger of a Glock and pull, and a round is chambered, it'll fire. If you pull the side of the main trigger without activating the safety trigger in the middle, it will be prevented from being pulled. this is different than some other models that have a separate button or lock away from the trigger area that needs to be released prior to pulling the trigger.

yeah, right.
It seems that things are really heating over this now.

A police station has been set on fire and a police officer and her family have fled as her house was surrounded by furious protesters.

Officer Kim Potter has now been charged with manslaughter over Mr Wright’s death.

Just finished watching it. Be interesting to see what the next episodes will highlight.
It did bring back many good memories of that time.
Sadly, it also brought back several bad ones.
Bear in mind that the night buses were 1 per hour.
Finishing a late shift this particular night, I managed to catch the last Circle & got off at Liverpool St.
Didn't have too long a wait for a night bus heading in the direction I wanted.
Got off at Dalston Junction & saw the night bus I needed in the distance, just missed it.
From there to where I lived was approx. 30 minute walk away, so 2 choices, wait 1 hour or walk.
I walked.
Being a very cold day, I was wearing my overcoat.
I was halfway down Graham Rd when I heard a voice.
"Where are you off to lad?"
Turned, looked, 2 coppers in vehicle.
"Home"
"Where have you been to at this hour of the night?"
"Working"
"Doing what?"
Rather than say anything unbuttoned my overcoat & pulled to one side so my uniform would be visible.
"Why didn't you say so in the 1st place?"
Oh, I forgot to mention that I was also carrying my equipment bag on my shoulder. It was all black but did have a worldwide famous symbol on the front.
The only difference between the worldwide famous symbol which is red, is that the one displayed on my bag was white (for obvious reasons of course).
Highly visible
We had "very" intelligent coppers them days. :-)
That was one of the reasons why I bought a motorbike. All good & well having free travel but when one finished a very late shift or did an early shift with the 1st train out of the depot approx. 4am, & the 1st buses starting approx. the same time, crossing from East to North London...
Edited for spelling.

Technology to the rescue. :-)
It took 3 to take the car to the station?
No wonder one can never see a copper around when they're needed.

Very good that this is being seen 'completely out in the open':

George Floyd death: Five key moments from the Derek Chauvin trial

The really hard part is how all that is judged and for what positive progress is then made.

Very glad I'm not the judge presiding over that one!

Despite the badly suspicious press, I think this is rather a good idea and a good example here with a good outcome:

NYPD Deploys "Creepy" New Robot Dog In Manhattan Public Housing Complex
wrote:
... Since October, the NYPD has dispatched the robot to a handful of crime scenes and hostage situations...

... A spokesperson for the NYPD said the robot dog was on standby, but not used, during a domestic dispute at East 28th Street on Monday afternoon. After a man allegedly barricaded himself inside a room with a mother and her baby, officers showed up and convinced him to let them exit. The man was arrested for weapons possession...

Fantastic addition to the team.

No "do or die" scary hair-trigger moments for the approaching police. No "Wild West shootout".

Very good result for facing what very well may have been a deadly gun wielding crazy shooter.

Derek Chauvin found guilty on all counts in the murder of George Floyd.

Chauvin, 45, was found guilty of second- and third-degree murder and second-degree manslaughter.

The judge asked each juror if the verdict was correct, thanked them for "heavy duty jury service," and dismissed them. He revoked bail and told Chauvin to report back in eight weeks for sentencing. Chauvin was handcuffed and taken into custody.

George Floyd: Chauvin conviction a giant step forward, says Biden
wrote:
... In televised remarks shortly afterwards, Mr Biden said that such a verdict was "much too rare".

"We can't leave this moment or look away thinking our work is done. We have to look at it as we did for those nine minutes and 29 seconds," he said.

Vice-President Kamala Harris urged lawmakers to pass the George Floyd bill aimed at reforming policing in the US.

"This bill is part of George Floyd's legacy. This work is long overdue," she said...

And yet unfortunately, there is this fraught encounter that looks all too ripe to be sensationalized badly:

Ohio shooting: Columbus police shoot dead black teenage girl
wrote:
A police officer in the US state of Ohio has shot dead a 16-year-old black girl while responding to an emergency call over an attempted stabbing...

... Columbus police have produced bodycam footage which appears to show the girl attacking others with a knife before being shot.

An inquiry is under way, with officials urging local residents to remain calm...

... Columbus Mayor Andrew Ginther described it as a "horrible, heart-breaking " and a "tragic day in the city". "We know, based on this footage, the officer took action to protect another young girl in our community - but a family is grieving tonight."...

How can the police hope to deal with guns and knives with a split second before 'do or die'?...

And how can stereotyping be avoided when seeing situations that are repeated?...

... There has to be a better approach all round.

Oh dear, how low can it go? :-(

US police officers who roughed up and laughed at elderly dementia patient resign.

Three Colorado cops involved in the brutal arrest of a 73-year-old woman with dementia — and later seen laughing and joking about the incident — have resigned, US police said on Friday.

Loveland Officers Austin Hopp and Daria Jalali and Community Services Officer Tyler Blackett stepped down over the June 2020 arrest of Karen Garner, Police Chief Robert Ticer said during a news conference.

“Our goal at the Loveland Police Department has always been to make our community proud. We failed and we are very sorry for that,” Ticer said.

Body camera footage allegedly showed Hopp forcing the elderly woman to the ground and Jalali slamming her against a police car, after she walked out of a Walmart without paying for about $US14 worth of items. Blackett helped transport Ms Garner to the station.

Video released by Ms Garner’s lawyer showed the cops laughing and joking about roughing up the woman — who reportedly suffered severe bruising, a broken elbow and a dislocated arm.

Both soldiers were interviewed by the Historical Enquiries Team (HET) in 2010 and it was this evidence that formed the prosecution's case.
11 years to decide to prosecute & 49 years after the incident.
Normal people know the law is the law. Time to remove the dildos applying it.
