Dallas air show pilots had no altitude plan before collision that killed 6, expert says
Chris Manno, a retired Air Force and American Airlines pilot, told Fox 4 his major takeaway from the report was that “there were no altitude deconfliction briefed before the flight or while the airplanes were in the air.”

“So they did not prepare for the eventuality of having two aircraft at the same altitude at the same airspace,” he said. “That’s a problem.”

According to the report, citing air show radio transmissions and aircraft surveillance data, the “air boss” directed the fighters to transition to a trail formation and fly in front of the bomber formation before proceeding near the 500-foot show line.

“However, they had an intersecting flight paths, and there was no provision made to say we will not be at the same altitude,” Manno told Fox 4.

A plane crashed Thursday morning in White Settlement near the Fort Worth Naval Air Station Joint Reserve Base and Lockheed Martin facility, according to police.

NBC 5 was told the pilot ejected, though his or her condition is not known.

Police told NBC 5 that US Navy Police and Lockheed Martin asked them to help clear an area near White Settlement Road and Spur 341/Lockheed Boulevard at about 10:15 a.m.

The type of plane has not yet been confirmed, though from Texas Sky Ranger it appears that a Lockheed Martin F-35 Lightning II came to a stop off in the grass off the southwest side of runway 18/36.

The F-35 fighter jet is assembled at the facility immediately to the west of the base.

Further details about the incident have not been confirmed by Lockheed Martin or officials at NAS JRB.

At least 19 people were killed and 32 injured when a fuel tanker exploded in a tunnel north of the Afghan capital Kabul, a local official said Sunday.

The Salang Tunnel, which is around 80 miles north of Kabul, was originally built in the 1960s to assist the Soviet invasion. It is a key link between the country's north and south.

A spokesman for Parwan province, Said Himatullah Shamim, said Saturday night's tunnel explosion killed at least 19 people, including women and children. He said survivors remain trapped under rubble and that the number of casualties could rise.

It was not immediately clear what caused the incident, which happened at around 8.30 p.m.

Parwan's health department has received 14 dead and 24 injured so far, according to local official Dr. Abdullah Afghan. There are five women and two children among the dead, he said, and the rest are men who are severely burnt and cannot be recognized.

A spokesman for the Ministry of Public Works, Molvi Hamidullah Misbah, said earlier Sunday that the fire was extinguished and that teams were still working to clear the tunnel.

Eleven people were seriously injured in the terrifying incident onboard a Hawaiian Airlines flight from Phoenix, Arizona, to Honolulu in Hawaii. 

Twenty were taken to hospital, including a 14-month old, a teenager and three crew members.

At least one person was reportedly knocked unconscious in the ‘isolated and unusual event’ on the Airbus A330-200. All patients were conscious when they arrived at the hospital.

Among the bystanders who helped rescue the family was Tampa Bay Buccaneers backup quarterback Blaine Gabbert.

Emergency services in Queensland are responding to a collision between two helicopters near Sea World, at Main Beach on the Gold Coast.

The collision occurred at 1.59pm local time (2.59pm AEDT), according to the Queensland Ambulance Service, which has multiple crews at the scene assessing at least 13 patients.

Images show both of the helicopters involved lying on the beach, having suffered serious damage, with emergency workers swarming around them. Each aircraft bears the words “Sea World Helicopters”.

There are fears that as many as three people may have died in the incident. The ambulance service said 13 people were being assessed at the scene....

The aircraft, which reportedly recently landed from Dallas-Fort Worth International Airport, was parked at the gate with its parking brake on "when a ground support personnel was ingested into the engine," the statement said. "The NTSB has opened an investigation into the accident."
