Whilst the dispatcher may get some blame, it's down to the driver to read all relevant road signs...

The 'typical unfortunate trick' is that:

  • Dispatcher doesn't know the route to know to specify a single deck bus;
  • Or there is not a single deck bus available that day;
  • Bus driver 'forgets' the bus is a double decker (very easily done!);
  • Or the bus driver deviates from the normal route...



Regardless, there has to be procedures and 'common sense' to avoid such crashes.

Note:

Personally, I am strongly against calling such crashes "accidents" or "incidents" that euphemistically ignores the consequences.


Frequently hit low bridges get electronic height warnings or sacrificial height barriers installed so that an unaware driver is very much loudly alerted before smashing into the bridge. However, that is expensive and usually only done after a few crashes...

Instead:

We already inexpensively have radar distance sensors for "braking assist / collision avoidance" 'safety' on cars. Require that be fitted to all high passenger vehicles to detect both other vehicles AND low overhangs?

In olden days (say, anything more than about 10 years ago), the local newspaper would have sent down a 'leg man' to herd up the kids and listen to their stories. Was this the normal route they took to school? Was it normally a double-decker bus? Did they know today's driver? You very quickly get to the bottom of the problem with questions like that.

Instead, the regional TV station rings up the bus company's PR guy and asks for a statement to camera. Nothing survives that treatment.

And why before people go for their written test, let alone their actual driving test, they should first be tested for stupidity & idiocy. Such people should never be allowed to drive a motorised vehicle.

She was treated at the scene by paramedics. No arrests have been made, police added.
You'd hope they'd at least charge her with committing a dangerous act or something similar.
I can't imagine how someone would have felt if they had run over her, even if there had been no way to avoid her.

‘Dreadhead Cowboy’ faces 3 charges after riding horse on Dan Ryan Expressway
At around 4:30 p.m., police responded to the southbound lanes of the Dan Ryan after Adam Hollingsworth, 33, better known as the “Dreadhead Cowboy,” was seen riding his horse on the expressway near 35th Street.

He rode for around 30 minutes as traffic slowed to a halt with Illinois State Police and Chicago police following close behind. Police said they asked Hollingsworth and those who were with him to exit the expressway multiple times and said they refused.

A couple of highlights are that the driver failed his post-accident alcohol test (and had marijuana present), and CSX were not implementing their own D&A testing policy adequately.

Thanks for the summary!

That does highlight that long distance driving is deadly boring and dangerously monotonous for most/many/normal(?) people...

Hence the scary number of times I've seen large vehicles drift across the motorway lanes, driver completely unaware, whilst making breakfast on the steering wheel or fumbling with whatever mobile device or... even watching TV! All whilst controlling lots of tonnes of 100kph metal momentum... (100kph+ downhill with the engine disengaged...)


'Tachographs' have helped... But what else is needed?

Aircraft-style monitoring?

Suicidal spike to replace the steering wheel mounted airbag?

A 32-year-old woman in a white Volkswagen Golf drove through barriers surrounding the elevated beachside car park about 10:25am this morning.
The car fell more than 20 metres off the cliff and landed on the rocky outcrop of Bar Beach below.

Antonov An-26 Military Plane Crashes in Ukraine, Killing at Least 22.

A military plane crashed during a training flight Friday night in the city of Chuhuiv in Kharkiv Oblast, a province located in eastern Ukraine. The country's emergency service said that 22 people are confirmed dead of the reportedly 28 passengers on board.

According to reports, there are two survivors who have sustained severe injuries, while the search is ongoing for four more passengers.

The plane crashed at roughly 9 p.m. and was immediately engulfed in flames, according to the Ukrainian police. The fire was reportedly put out by 10 p.m., but the search for the unaccounted passengers continues...

..The army reported that the AN-26 military aircraft is used to carry students of the Kharkiv Aviation University of the Armed Forces of Ukraine.
