I thought your NHS was the Bee's Nees of health Care . Man, is this STILL going on? Whisky Tango Foxtrot?!?!?!? 40 to 50 YEARS ago: 1977... Norristown, Pennsylvania A few years before that, a hospital in Pasadena, Texas (Southmore Medical Center -- newly built) had this happen... The FIRST baby born at the hospital, well it wasn't the baby that died, it was the mother. Error in labeling the outlets between the Oxygen and the Nitrous Oxide. 4 years earlier, in Ontario, Canada, the same mistake happened but with 14 deaths. Damn. Thought everywhere OX is left hand thread. But if you have a connector that doesn't have a thread. Hell Bells, do they need a building code that requires all OX lines and connectors to be some non-compatible (not metric or English) size so they can't be crossed? Because whatever is being done now isn't enough. |

Thought everywhere OX is left hand thread Oxygen left hand thread ??? 20+ yars of working with industrial gas cylinders ( welding /cutting)left hand thread is for propane ,Acetylene etc .never come across a left hand thread Oxy cylinder BOC , Air Products ,Linde and air liquide cylinders are all the same fittings and they are the big 4 main suppliers. Because whatever is being done now isn't enough. Sure... But.... When the contractor that built the building accidentally/negligently crosses the lines INSIDE THE WALLS... Pfizer fined £84.2 million I find something wrong with this report & it's not about overcharging! Phenytoin is still being dispensed? That drug was developed in the 1930's & by 2005 was being phased out as it has drastic side effects. After her discharge in 2005, my partner was prescribed that along with Kepra (brand name as the medical name is too long & awkward to pronounce). By the summer of that year, they stopped prescribing Phenytoin for her. The Kepra wasn't enough to stop her having fits so after letting her consultant know that, he informed me about why Phenytoin was being phased out. He then added Lamotrogine which worked a treat. Why is Phenytoin still being prescribed? |
