A tool for making the lift up flap used to insert a cornea, some sort of microtome perhaps?

Yes it is a Microkeratome device for cutting a flap exposing the cornea before LASIK eye surgery.

From the clues and comments, I presume a dentist's tool.

Does this item have a single tool, i.e. the pointy end, or are the metal loop and the small hook near the cylinder also used?
