Ahh ... Got it :-)

The arena formerly known as Northlands Coliseum, Edmonton Coliseum, and Skyreach Centre. Currently named Rexall Place. It's in Edmonton,Alberta and the Great Hockey Player name Drive that Gary mentioned in the psuedo clue is Wayne Gretzky Drive NW. Please give the win to Gary as he clearly knew the answer before me (unless, of course, my answer is wrong).

edit: Don't want Mr. Gretzky angry with me for misspelling his name.

USA - Yes.

Focal length - Certainly not a $160,000 lens ;)
Can't say for sure as it's not my photo and EXIF Data on the original is missing. My guess would be no, not extreme, as there doesn't appear to be much distortion. Probably no shorter then 28mm, but that's my opinion.

Man made - It's a real geological feature but there was some unintentional assistance with the initial formation. As far as I can tell, the photo is real and not 'shopped. I did check other images from other sources and the appearance is the same.
