Senior engineer reported to management for failing to fix a stapler One of the quasi-governmental organizations I did IT support for instituted a policy whereby anyone could escalate any IT ticket up to "severity one" highest priority, which automatically triggered a broadcast to IT and a conference of the incident co-ordinators, among other things. Predictably, this policy lasted approximately one week before it was cancelled, because a mid-level exec. invoked the a severity one IT incident process as he didn't think that the plant in his new office matched the other décor.

 The number of people at work that have difficulty with this concept is rather concerning (although many of them don't comment until there is nothing left at all, let alone almost nothing- and that's for pens, rubber bands, clips pretty, much everything).
