The accountants will tell you spending a billion or so, on more safety is a waste of money, when the cost of insuring to cover the costs of the few accidents, is only a few million/year.

China air crash that killed 132 may have been deliberate, says US report
WSJ sources suggest black box recorders from Boeing 737-800 indicate intentional input from cockpit

A China Eastern Airlines plane that crashed in March, killing 132 people, appears to have been intentionally flown into the mountainside below by someone at the controls, according to reports.
Analysis by US officials of the black box flight recorders found amid the wreckage suggests deliberate input from the cockpit forced the Boeing 737-800 plane into its catastrophic dive.
The Wall Street Journal quoted an unnamed source who said: “The plane did what it was told to do by someone in the cockpit.”
The China Eastern plane was cruising at a steady altitude and speed before it suddenly descended more than 20,000ft in just over a minute, and crashing near the city of Wuzhou in Guangxi province.
Chinese investigators have led the crash inquiry but US officials are involved as the plane is US-made.

Yes, and sadly having actual footage of that crash really left it beyond doubt that it was a controlled dive overriding both aerodynamic principles and electronic fail safes. :-(

Having been involved at the sharp end pf the compliance industry....
It is not uncommon for early issues of so called compliance demonstration documents to be both inadeqiate and incomplete.
Why do I say both?
Incomplete means they don't cover all the required clauses of the standards.
Inadaequate means that the responses to clauses does not clearly and unequivocally demonsttate compliance with one or more clauses of a given stamdard. It's no good saying "We comply" without the appropriate demonstration of compliance.

Boeing said July 27 it will take another charge against its earnings because of the CST-100 Starliner commercial crew program as the company and NASA get closer to a first flight of the vehicle with astronauts on board...

... “driven by launch manifest updates and additional costs associated with OFT-2,” the second uncrewed test flight of the vehicle that took place in May. The company did not elaborate on the specific issues that caused the charge...

... The [Crewed Flight Test] CFT mission, NASA announced then, will last two weeks after earlier proposing to keep it at the station for up to six months. A two-week mission, the agency said, “is sufficient to meet all NASA and Boeing test objectives for CFT,”...

... If CFT is successful, Starliner could start operational missions as soon as the fall of 2023, after the SpaceX Crew-6 mission launching in the spring of 2023...

Boeing slammed with another FAA airworthiness directive on 777 aircraft

Second directive

The recent directive is the second in less than two weeks after the FAA voiced concerns about potential electric discharges in the aircraft type’s fuel tanks in case of a lightning strike or electrical short-circuit. The directive issued last week mandates operators to install mitigation materials, and perform detailed inspections and corrective actions...

Reducing the Starliner CFT mission from six months down to two weeks is curious... Are there lingering concerns about corrosion accumulating to cause the thruster valves to jam/fail?...
