Titan sub disaster was caused by weak safety and oversight, Coast Guard says
The Titan submersible disaster could have been prevented, the U.S. Coast Guard said in a report Tuesday, but OceanGate CEO Stockton Rush ignored safety warnings, design flaws and crucial oversight that may have resulted in criminal charges had he survived.

The report found the company's safety procedures were "critically flawed," noting that the core of the failures inside the company came down to "glaring disparities" between their safety protocols and actual practices.

I guess time will tell once the bridge is constructed and in use how sturdy it is.

Karma strikes again.

Footage has captured the moment a hit-and-run getaway backfired on a busy Melbourne intersection.

A driver’s reckless attempt to flee a hit-and-run has ended in a spectacular crash at a busy intersection in Melbourne last Thursday.

The chaos began around 6.20pm, when a driver in a grey Kia suddenly swerved in front of another driver while attempting a U-turn in Hampton Park, in the city’s south-east.

The startled man, who filmed the entire incident on his dashcam, shouted “what the f**k” and “pull over” while repeatedly flashing his headlights towards the erratic motorist.

The situation escalated moments later when both drivers pulled up to a red light. The man can be seen exiting his car in a bid to confront the driver of the Kia who in response, crawls closer to the busy intersection.

The attempted getaway quickly turned disastrous when the Kia sped through the red light, colliding with a white car between Hallam and Pound Street.

Footage then shows the Kia, in what feels like slow motion, flip onto its roof while lanes of busy traffic pass.

Thankfully, bystanders confirmed that “everyone was fine after it.”......
I wonder what her excuse to the judge will be.

I wonder what his excuse will be for his totally OTT violently threatening behavior against another motorist, and for subsequently directly causing a potentially deadly crash?

Very stupid all round.


(Much better, and safer for all, is to simply report whatever concerns to the police. And have a nice day.)

Can't believe we watched the same clip. I saw the cam driver with a green light and the other car run a red and got clipped. Then the cam driver wanted to do the correct thing and stop and exchange information. Other driver didn't want to for whatever reason and ran causing another collision.

Italian engineers and construction companies are superior to all others. They know how to remove the debris from a suddenly collapsed large bridge and to construct a replacement within just 23 months.

St. Lucie County – On August 12, 2025, State Troopers responded to a crash on Florida’s Turnpike occurring in St. Lucie County. The crash involved a mini-van and a commercial semi-truck and trailer. Based upon our initial, but on-going criminal investigation, it is evident that the driver of the commercial semi-truck recklessly, and without regard for the safety of others, attempted to execute a U-Turn utilizing an unauthorized location. As a result of his actions, the three occupants of the mini-van are now deceased.

Multiple people dead after funicular crash in centre of Lisbon.

At least 15 people are dead after a funicular cable railway went out of control and crashed in the centre of Lisbon.

A vehicle on the Portuguese capital’s famous Elevador da Glória derailed and hit a building, authorities stated.

The incident happened at around 6pm on Wednesday afternoon, local time (2am, Thursday, AEST). At least eighteen people are injured.

The SIC television channel quoted a person as saying that the funicular hit a building as it sped down the steep street “at full speed.”

“It hit a building with brutal force and collapsed like a cardboard box; it had no brakes,” the woman said.......

Wild footage shows the moment a $1.4 million super yacht starts sinking just moments after taking to the sea, with a man seen jumping overboard.

This is the jaw-dropping moment a luxury yacht capsized and sank just moments after it was launched on its maiden voyage.

Staggering footage shows the £700,000 ($1.4 million) boat hitting trouble just minutes after taking to the sea.

The astonishing incident took place off the coast of Zonguldak – a popular tourist spot in northern Turkey – on September 2.

A clip shot on a phone captures the moment the vessel tipped over into the deep.......

.......Rescue teams have also been dispatched to begin recovery work.

Shipyard officials said an investigation will be conducted to establish the cause of the sinking.

While the probe remains ongoing, experts have suggested that stability problems, caused by errors in calculating the boat’s metacentric height, are the most common factors in these type of incidents.

It looks like it was stern heavy- the camera angle isn't right to see what was going on there, but it looks like when it went in the stern was too low, and it took on water there (you can see the bow getting higher and higher) until it filled up enough to affect the trim, and over she went.

Still being reported as 15 dead and 18 injured (many serious), police investigating.

there's been at least one previous derailment (2018?), but no reported serious injuries. It's been reported that the 2018 derailment was due to the lack of maintenance:

According to The New York Times, the Lisbon Firefighters Regiment said the accident was caused by “a cable that came loose” in the structure of the funicular.
Horrific... a loose cable. This should never happen. Impossible. It's not about a "derailment" which just would lead to insignificant wheel and rail damage... Regular maintenance normally should ensure sufficient safety margins for cable wear and mountings never risking a broken cable or cable connector.

It's unfortunate that the existence of such historic railways is threatened today simply because their maintenance is sorely neglected. This must be painful for generations of funicular operators and mechanics... The funiculars in Lisbon or San Francisco, for example, are world-famous and tourist magnets. No money? Really?

We have two similar historic cable cars and cable-run suspension railway in Dresden (Saxony). They depend on an active community of enthusiasts keeping them in shape... and frequent maintenance... ah and sufficient money for inevitable replacement of worn heavy machine parts.

When asked if it was being investigated as a mechanical failure, TfL said the investigation was ongoing and they would not be able to add anything to their statement at this time.
