Not quite, the "technology" hadn't failed this time, but the training was most certainly lacking and misleading. One issue with many modern cabs is that they are liable to encourage a "head down" driving style instead of an "eyes out" one. Had this driver used his old skills instead of trying to get his round a poorly structured menu and display system this accident would probably not have happened as he would have been watching the train ahead of him.
Also of interest is this driver had a couple of SPAD events in his driving career, so was possibly less observant or more liable to distraction than desirable?

Agree even though it was never said that technology failed, it highlights just how much technology is involved. I have no idea how mainline works then or now, during my time on the trains.
They were tested & checked for operation in the depot prior to entering passenger service, so once in service, it was always a case of "heads up".
Unlike today, those days, the important control panels (non driving controls) were not low down taking one's view away from a heads up position.
Also, permissive working was not possible due to the way signalling operated.
There were several instances where trains collided on the same section but in each case it was shown that the driver had isolated (disabled) a major safety feature that enabled those collisions to occur.
As for those SPAD's, both occurred in the 2 years he had reduced driving time, that in itself was a clear warning something was not right, especially in light of his extensive experience.

Train crash: Driver's struggle with software 'led to collision'
So... What could possibly go wrong at 5mph in a train depo?...

That outage sounds expensive and very much like an all-too-typical "operator error".

That also highlights the importance of clear training (especially for that train) and for testing the driver's competence.
Yup, that's what it sounds like at first - just a parking lot shunt.

But have a read of the full 55-page Rail Accident Investigation Branch report - they've really thrown the kitchen sink at this one, with global implications. And no - it wasn't just operator error, whatever the BBC sub-editor thinks.

It's good to see that the RAIB, at least, is still doing its job properly.

Thanks for that. That is a very well put together report. Beautifully no sensationalism or blame.

Note that the immediate circumstances for the crash developed during 27 seconds whilst the driver's attention was focused on the computerized Train Management System to undo a non-standard 'work around'...

However, that crash scenario was a consequence of 'out of the mundane normal' events from earlier and a good reminder of how a series of minor problems/annoyances can add up to disaster.


That is also a very sharp reminder and example of how very small differences in operation can be unexpectedly disastrous.

For this railway example, the differences in operation between the different trains that driver operated that day look to be scarily much less than the differences across the supposedly "same type" Boeing 737 aircraft versions... IIRC: Part of the confusion that led to the deadly Boeing 737 crash at Kegworth was that the pilot had very good experience of flying the Boeing 737 yet was totally unaware that the version of B737 he was flying that day had the air bleed operating from both engines rather than, as the long experienced pilots had expected, from one side only...

...don't forget the manual. It implied that hitting "home" acknowledged the stops when in fact one needs to hit "Check Stops" for the TMS to accept it.

Amongst other design flaws.
Thinking back on permissive working. Again, no idea of modern rolling stock or even modern signalling systems, but the only such working we had was once we had left the main line to enter depots. Take Neasden for example.
IIRC, it has 58 roads with each road holding 4 8 car trains. With the depot divided into North & South, that was 2 on each road. There were several instances of "kissing". One very rarely saw A60/C69 (Met & Circle) & 38 (Bakerloo) stock on the same road. The rare occasions when one did was when the depot was practically full - mainly due to strikes. At any one time, there was only the one train entering the depot from the main line. However, one had to be aware of moving trains - shunters at work.

Video of A 14-year old stole his Grandfather’s car and was driving like a maniac in a children’s park, when the police started giving chase. Small kids were diving out of the way and it was an extremely dangerous situation. An alert good Samaritan in a big Ram ended it all!

... Kresen made a number of calls to 911 asking for assistance and officers responded. However, his car was buried under so much snow, they couldn’t locate it. Making matters even worse was the fact that the serpentine belt was broken, meaning Kresen was trapped without heat.

In a miraculous turn of events, Cawley hit the windshield of Kresen’s car while digging to check on the mailboxes. He then freed the man after being trapped in his car for more than ten hours. Following the rescue, Kresen was taken to the hospital to be treated for hypothermia and frostbite.

People may or may not remember accidents involving Teslas where the drivers said it was due to the car suddenly accelerating by itself, and a person bringing a petition that all Teslas made from a particular date to the present should be recalled.

After a 12 month investigation the NHTSA has finally released it's report. And it's findings? In all of the occurrences of "sudden unintended acceleration (SUA)" that it was able to investigate, every single one was due to the driver- using the wrong pedal.

Drone operator admits to nearly crashing a California police helicopter, feds say
... Hernandez admitted in the plea agreement he was curious on a night in September when he heard police vehicles driving down the street and a helicopter flying above his Hollywood home. In search of an answer, he launched a drone toward the police activity, authorities say.

An L.A. police helicopter pilot responding to the emergency spotted the drone and unsuccessfully tried to avoid it. The drone hit the fuselage of the helicopter, forcing an emergency landing.

... Hernandez could go to prison for up to a year. His sentencing is set for April.

7 Hurt After NYC Tandem Bus Veers Off Road, Leaving Front Half Dangling Off Overpass
At least 7 people were injured after a New York City accordion-style bus veered off the road and half of it dangled off an overpass.

A police spokesperson says the BX-35 bus driver was "unable to navigate the roadway and went off the road" near the Cross Bronx Expressway and University Avenue around 11 p.m. Thursday. No other vehicles were involved in the incident and no arrests have been made.

Six of the injured were bus passengers and the other was the driver and they were transported to the hospital with minor injuries, according to police and MTA spokesperson Tim Minton.

The FDNY later said there were nine people injured in total. It's unclear if the other two are also passengers.
