A guy I know works for Siemens and was involved in the delivery of the Chargers to both Washington and Illinois DOTs. He heard about this within 15 minutes and was immediately dispatched to the scene as their rep. All he is willing to say publicly is that (as I myself thought from the pictures) the locomotive cab did its job of protecting its occupants.

I also received an email from the Railroad Passengers Association stating that two of the three fatalities were members, one a board member. No doubt they were among the several railfans who were riding just because it was the first revenue trip over the new cutoff route.

It reminds me of an similary accident in Spain not long time ago.
The driver ignored or failed to keep the speed limits.

The train in Washington was travelling at 80mph (130km/h) on a curve with a speed limit of 30mph, data from the train's rear engine indicates.

If a road is covered in snow & ice, do you drive the way you usually do, or take a bit more care?
Naturally, drive the way you usually do.

OMG, whatever will they think of next? Lawyers will be loving that! All those "accident" claims.

Maybe they could invent a common carrier with a universal service obligation - put all the packages on a single van, or a succession of vans running on a fixed interval to a known timetable. It works for buses.

If you need an emergency dentist you should park your car first and enter via the front door.

Pegasus Airlines said the plane "had a runway excursion incident" as it landed at Trabzon

Yeah, a particularly impressive boo boo that one.
Very lucky there were no fatalities.

16 passengers are now listed as being taken to hospital, but thankfully no serious injures were incurred.

Cheers.

Train crash near Milan, Italy, leaves at least three dead after derailment.
At least three people are known have been killed after a train derailed, folding one carriage in half.
At least 100 people have been injured, 10 seriously, but the death toll could rise as there were still people trapped inside.

Train wreck of a local train between Pioltello and Segrate, near where I live. Three people dead, 5 seriously wounded, about 100 slightly wounded. The train belongs to Trenord, the Railways of Regione Lombardia, the tracks belong to the State railways, so they are already quarreling about the responsibility, as usual in Italy. The Milan-Venice Treno Alta Velocita' is also interrupted.
