Has anyone had this happen before? Had Dual 460's and the fan gave out on this one. Looks like an RMA..

Looks much the same as the first one I got - it let go after about 10 minutes with what might be described as an "interesting" sound....

Not on an EVGA, but on a Palit 768 MB GTX 460 I managed to snap off some fan blades by sticking my fingers into them while trying to sense the thermal performance -- I didn't even feel the blades hit my fingers, they were so fragile. Card is now force-cooled in a Supermicro 1U chassis designed for two GPUs (with a baffle made from a Rice Krispies packet to get optimum cooling) and is helping my RAC.
Also had a Gigabyte GTX 460 fan fail recently at less than a year old -- I RMA-ed it to eBuyer, but because it's discontinued I paid the three-quid difference to get a GTX 560 replacement.

I don't know how much 3 quid is, but it sounds like a good deal to Me, turned a broken 460 into a 560, that's an inexpensive upgrade.
