We have a winner. It is the drive gear from a cog locomotive. Probably a Riggenbach style.

Your both right that it has a cam, however its not from a vehicle.

Your part right, the machine it's from was multipurpose, it was used by farmers, construction and industry.

All right, sorry about the long wait. This gear is not from a sewing machine or pile driver. HINT the machine also has 2 flywheels.
