The default sorting function on the forum pages should be "Most recent answer first" and not "Most frequently asked first".
A lot of people are asking the same questions as they do not pay attention how the sorting is made.

When displaying "Most recent answer first" should there be times in the "answered" column for all the recent entries? Some show times, while others show "--"?

The ones that show -- are the questions that have not yet been answered, so they have no time for the most recent answer.
